To order by cheque:
Please complete the Order Form that we have mailed to you and return it along with your cheque.
To order by credit card:
- You will need to enter your Customer Number (example: E-001234 or R-001234) when you fill out the payment page. You will find your customer number on the top-left corner of the label that is attached to the Order Form that we have mailed to you.
- Click the Buy button below to display the payment page hosted by Stripe, or use this link to the payment page.
